Arkansas Better Chance (ABC) Program

 The following items are required for registration:

• Child Physical Form

• Birth Certificate or Hospital Record

• Child's Social Security Card

• Child's Insurance Card

• Child's Pick-up List

• Immunization Record

• USDA Food Program Eligibility Form

• Proof of Income: Total Family Income

All Parents/Guardians are required to provide the following for your students:

• Two (2) Change of clothes available for
  your child(ren) each day.

• Two (2) blankets for naptime.
